Micro Tese

What is Micro Tese and How Is It Made?
Micro tese is the process of obtaining sperm from a tissue sample taken from the testicles by microscopic operations in men who do not have any sperm cells in their semen. Males must have non-occlusive azoospermia in order to perform the micro-tese procedure. This procedure is performed under general anesthesia. During the procedure, the testicles are opened and the sperm channels are observed. Then, the most developed full sperm channels are determined and sperm is tried to be obtained from these channels.

Up to 70% of sperm can be obtained successfully in patients with azoospermia diagnosis. The ability to obtain sperm is directly related to the underlying cause of the diagnosis of azoospermia. Micro tese procedure can be applied in cases of male infertility. In men who do not have any sperm in their semen, sperm is obtained and a successful pregnancy is achieved with in vitro fertilization treatment. Because this process allows healthy sperm to be obtained.

Micro tese process has been used in the world since the 90s. Although there is sperm production in the testicles, if there is no ability to be expelled, the person may not have a child. With this process, sperms taken directly from the testicles are put into the egg and a successful pregnancy can be achieved. Micro-tese is frequently used in IVF treatment.

How is Micro Tese done?

Micro-tese procedure is performed under general anesthesia. During the surgery, the tube particles containing the sperm are first cut and removed. These parts are then examined in the laboratory environment. During this process, live sperm are separated. The healthy and viable sperms obtained are combined with the egg by microinjection method.

Since the surgical method is applied in the micro-tese procedure, the risk of bleeding is very low. Less tissue is removed during the procedure. This surgery takes between 1-2 hours. Factors such as the patient's condition and the number of sperm ducts to be examined are determinative in the duration of the operation.

The sperm obtained by this method can be stored for later use. The patient is discharged immediately on the day of surgery and can return to his daily life. After the operation, the patient is kept under observation for only a few hours and rested.

Less sperm can be obtained than the sperm required during the procedure. In this case, a treatment can be applied to increase the sperm count of the person before micro-testing. It is possible to find 20% sperm after sperm augmentation treatment in people who do not have enough sperm in the first procedure. The tissue taken during the surgery is genetically examined.

What should be considered before micro-tese?
With micro-tese, healthy sperm can be obtained from male candidates with infertility problems by microscopic examinations. Before this procedure, there are some points that patients should pay attention to in order to increase the success rate.

Before micro-tese, the patient does not need to follow a special diet. Because the sperm search process is done directly on the testicles. Some patients may have a small number of sperm in the semen. In these cases, the patient's semen is taken before the procedure. After this procedure, micro-tese application is performed. Patients with insufficient sperm count in their semen should not have sexual intercourse for a certain period of time before the operation.

There is no sexual intercourse abstinence in patients with complete azoospermia diagnosis. Micro-tese is considered a surgical procedure. Therefore, before the operation, the patient should stop the blood thinners 7-10 days before the operation, within the time specified by the doctor. Those diagnosed with systemic diseases such as diabetes and blood pressure should talk to the doctor about their health problems and should follow the points specified by the doctor in order not to adversely affect their health.

What should be considered after micro-tese?

Micro-tese is an extremely easy procedure. After the operation, there is nothing that patients need to be very careful about. However, in the name of personal health, it is useful to pay attention to the points stated by the doctors.

The following points are important after micro-tese;
In order for the patient to be discharged, the effect of anesthesia should be waited for. This effect will pass within a few hours.
Since the patient will be a little tired after the operation, he should not drive on the way home.
After micro-tese, the patient should not have heavy physical activities for at least 10 days.
You should not have sexual intercourse for 10-15 days. The time recommended by the doctor should be waited for sexual intercourse.
After micro-tese, dressing should be applied to the treated area. For this, the hygiene of the area should be ensured. If hygiene is ensured, the risk of infection is eliminated.
Patients can start taking a shower after 2-3 days. During this process, the area should not be touched by water.
The patient should not wear tight underwear for up to 15 days. This is directly related to the comfort of the patient.
After the operation, the area is stitched. However, the sutures usually dissolve on their own. Therefore, there is nothing to be done to dissolve the seam.

What are the risks of micro-tese?

Micro-tese is an operation that has no side effects when done professionally. Postoperative patients may have complaints of pain and bleeding. However, if you pay attention to what the doctor says, these risks will disappear.

First of all, since this procedure is a surgical operation, there are many risks such as bleeding, pain and infection that can be seen in surgical operations, albeit low. However, micro-tese is generally a painless and harmless procedure.

Can micro tesing be done more than once?
Micro tese is an application in which it is possible to obtain sperm in case of infertility in men. Even if sperm is obtained in this process, pregnancy may not be successful in IVF treatment. In this case, a second application may be required.

In a person whose sperm is obtained once, there is a 90% success rate of obtaining sperm again in the second application. The underlying cause is decisive in this case. In some cases, although sperm can be handled in the first application, sperm may not be obtained in the second procedure.

A second application can be made in people who have failed the first micro-test. If the first application was unsuccessful, the second application has a success rate of around 30%.
